• Contact details you provide — your name, mobile phone number, and email address when you call us, send a message, request information, or begin booking.
• Appointment and intake information — collected by our scheduling and electronic medical record provider (IntakeQ/PracticeQ), not by this website.
• Site-usage analytics — pages viewed, approximate (anonymized) location, device type, and browser, collected through Google Analytics 4.
• SMS opt-in records — if you opt in to text messages, we (through IntakeQ Engage) record the phone number you provided and the time and method of your consent.

We do not knowingly collect medical or health information on this website.

How We Use Your Information

We use the personal information described above only for the following purposes:

• To respond to your calls, messages, and requests for information.
• To schedule, confirm, reschedule, or cancel your appointments.
• To send appointment reminders, confirmations, missed-appointment follow-ups, recall reminders, and other practice updates by SMS or email (with your consent).
• To verify insurance eligibility and process billing for your care.
• To understand how visitors use the website so we can improve it.
• To comply with applicable laws and protect the security and integrity of our practice.

Who We Share Your Information With

We do not sell your personal information, and we do not share it with third parties for their own marketing. We share information only with the following categories of recipients, and only as needed to operate the practice:

• IntakeQ / PracticeQ (EMR and patient communication, including Engage): for scheduling, intake, patient records, and appointment-related messaging. Covered by a HIPAA Business Associate Agreement.
• SMS / messaging carriers and clearinghouses: to deliver appointment-related text messages you have opted in to receive.
• Insurance payers, clearinghouses, and our billing provider: to verify coverage and process claims for your care.
• Google Analytics: for de-identified website usage statistics only.
• Law enforcement, regulators, or other parties when required by law, court order, or to protect rights, safety, or property.

SMS consent is not shared with third parties or affiliates for marketing purposes. Your mobile number and the fact that you have opted in to receive text messages are used only to deliver the messages you requested, and to comply with carrier and legal requirements.

SMS / Text Messaging

With your consent, we send appointment reminders, confirmations, recall reminders, and practice updates via SMS and email through IntakeQ Engage. Message and data rates may apply; message frequency varies. Reply STOP to opt out at any time, or HELP for help. Consent is not a condition of receiving care. We do not sell or share your mobile number, and SMS consent is not shared with third parties or affiliates.

Booking & Patient Records (HIPAA)

Scheduling, intake forms, and patient medical records are handled by our EMR, IntakeQ/PracticeQ, under a HIPAA Business Associate Agreement. Our handling of protected health information is governed by our practice’s separate Notice of Privacy Practices.

Cookies & Analytics

We use Google Analytics 4 to understand how visitors use the website. GA4 may set cookies and processes data such as pages viewed, anonymized IP / approximate location, and device type. You can opt out via your browser settings or Google’s opt-out tools.

Your Choices

Contact us to access, correct, or delete personal information you have given us, or to opt out of communications. You can stop SMS messages at any time by replying STOP.
